Enhance your overall well-being with a dedicated Brookdale RN Care Manager overseeing your clinical needs, including appointment scheduling and communication management with you, your providers and Brookdale. Many times, we can coordinate for providers to come to you so you can say goodbye to lengthy waits in doctors' waiting rooms and hello to more time for the things and people you love.
We facilitate onsite preventive care services such as routine screenings, vaccination clinics and annual wellness visits. This not only saves time but offers you peace of mind knowing that Brookdale HealthPlus communities are helping you manage your health care needs.
Technology in Brookdale HealthPlus communities better enables staff to monitor changes in your condition, helping to improve our ability to manage and respond to your physical, clinical and dietary needs. Together we can help you stay ahead of your condition and help keep you out of the hospital.
The Brookdale HealthPlus Registered Nurse (RN) Care Managers offer clinical oversight for your care plans, facilitating communication and coordination of care between Brookdale, you and your physicians. With everyone on the same page, you can get the quality care you deserve.
